Membership terms and agreement
Acknowledgement of risks, injury and obligations
I acknowledge that the activity I am to undetake is a dangerous activity and that by participating in it, I may be exposed to certain risks. I acknowledge and understand that while particpating in such activity:
1. I may be injured, physically or mentally, or may die
2. My personal property may be lost or damaged
3. Other persons participating in such activity may cause me injury or damage my property
4. I may cause injury to other person's or damage their property
5. There may be no or adequate facilities for treatment or transport of me if I am injured
6. I assume the risk of and responsibility for any injury, death or property damage resulting from my participation in the activity

Release and Indemnity
I participate in the activity at my sole risk and responsibility. I release, indemnify and hold harmless San Bu Kai Martial Arts Charitable Trust, its servants and agents, from and against all and any actions which may be made by me or on my behalf or by other parties for, or in respect of, or arising out of an injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever.
I also agee that in the event I am injured or my personal property is damaged, I will bring no claim, legal or otherwise, against San Bu Kai Martial Arts Charitable Trust in respect of that injury or damage.

Conditions of Membership
1. Appropriate covered footwear and a shirt must be worn in the fitness lab/studio. Towels must be used at all times. All weights must be put back after use. Children under 13 are not permitted in the fitness lab/studio.
2. Memberships are not transferable or refundable.
3. Guests of members who enter the facility must be made known to the instructor.
4. Members must respect other users and behave in an appropriate manner at all times by listening to and following instructions from instructors.
5. San Bu Kai reserves the right to rescind the rights of members not complying with the terms and conditions of the membership.
6. A parent/guardian must accompany their child(ren) to the entry door of the facility and pick them up from the facility.
7. Failure to keep membership fees up to date will result in the membership being terminated.

Privacy
We do not provide any of your details to third parties. All details remain confidential to San Bu Kai and are used to keep you updated on activities/events and facility closures.
